SIR – While visiting my local library I was asked to complete an adult-user survey form, a tick-box type questionnaire asking about quality of service, standards and facilities, staff helpfulness – the usual things that a service provider should know anyway.

The last two questions were rather different. What is your religion and what’s your sexual orientation? Both have opt-out boxes suggesting that the questions should not have been asked in the first place.

What does a request for what many would see as sensitive and personal information have to do with providing a municipal library service?
