POLICE are concerned the the health and welfare and a high risk Bradford teenager who has been missing for 12 days.
Jacob Lee, 17, was reported missing on April 2.
West Yorkshire Police have issued a high risk missing person appeal to try find Jacob.
The force is concerned due to his age, the length of time he has been missing and his health and welfare.
Jacob is described as being 5ft 4ins tall, of slim build, with brown curly hair.
He is thought to be wearing all black clothing.
