BRADFORD has been ranked the fifth best area nationally for how health and social work partners are performing together.

The Department of Health and Social Care and the Ministry of Housing, Communities and Local Government have brought together six key indicators of performance to assess each local area.

Coun Sarah Ferriby, Bradford Council's Executive Member for Healthy People and Places, said: "This is great news for Bradford and we are proud to be in the top five of all local authorities in the country managing effective and timely transfers from hospital for people who need social care support.

"This has been achieved against a backdrop of a step change in performance with an exceptionally strong rate of sustained improvement in our Adult Social Care CQC inspection ratings for providers in Bradford."

"Against the national and regional trends we have seen a 10% increase in the number of providers rated good or better by the CQC to 68.2%."

Dr Dave Tatham, GP and clinical lead for urgent and emergency care at Bradford District and Craven CCGs said: “This is excellent news for people living in Bradford and reflects how health and social care organisations are working together cohesively and effectively to provide care in (or closer) to people’s homes.

"Examples of multi-disciplinary working in Bradford include the Intermediate Care Hub which provides health and social care to patients in their community to help prevent them being admitted to hospital, along with the Multi-agency Integrated Discharge Team (MAIDT) who proactively assess patients social care needs early so they can be discharged from the hospital promptly when fit.

“Health and social care partners performing well together means that when people need access to care and support it is available to them, designed around their needs and as close to where they live as possible so they remain happy, healthy and at home.”
