Patients in Bradford should receive more efficient care when a system starts across the district.

Health and social care partners across Bradford and Airedale have been successful in a bid to NHS England’s Safer Hospitals, Safer Wards Technology Fund.

Connecting Health and Social Care in Bradford, Airedale, Wharfedale and Craven will make the district one of the first to join up health and social care information about each resident.

The project is led by Bradford District Care Trust in partnership with Airedale NHS Foundation Trust, Bradford Council, Bradford Teaching Hospitals NHS Foundation Trust and the district’s three clinical commissioning groups.

It aims to provide the right care in the right place, first time and will help support work to prevent hospital admission, long-term residential care or unnecessary dependence on the health and social care system.

Medical director at Bradford District Care Trust Andy McElligot said: “This is good news for local health and social care.”
